I hereby grant LTSC Community Development Corporation (also known as Little Tokyo Service Center or LTSC) permission to use my name and photograph in any of their publications and/or electronic media without payment or other consideration.
I understand and agree that these materials will become the property of LTSC and will not be returned. I hereby irrevocably authorize LTSC to edit, alter, copy, exhibit, publish or distribute photographs in which I appear for purposes of publicizing their programs or for any other lawful purpose.
In addition, I waive the right to inspect or approve the finished product in which my likeness appears. Additionally, I waive any right to royalties or other compensation from the use of the photograph(s). I hereby hold harmless and forever release LTSC from all claims, demands, and lawsuits which I, or persons acting on my behalf.

In consideration of being allowed to participate in Little Tokyo Service Center’s volunteer program and related events and activities, the undersigned acknowledges and agrees that:
1. There is a risk of injury from the activities involved in this volunteer program.
2. I assume all such risks, both known and unknown.
3. I willingly agree to comply with the stated and customary terms and conditions for participation in the volunteer program.
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless LTSC Community Development Corporation, its officers,
officials, agents, and/or employees, other volunteers, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct events (“Releasees”), with respect to any and all injury, disability, death, or loss or damage to person or property.
5. By signing this agreement, I waive my right to bring any legal action now and in the future to recover compensation or obtain any other remedy for any injury to myself or my property
or for my death, arising out of my participation. I further agree that, my spouse, assignees, heirs, guardians, and legal representatives will not make any claim against, sue or attach the
property of Releasees, for any loss or damage resulting from my participation, I agree if any claim for personal injury or wrongful death is commenced against Releasees I will defend,
indemnify and hold harmless Releasees from any and all claims or causes of action that are caused directly or indirectly by my participation.
